Memorial and Tribute Giving
Some of the most meaningful gifts Children's Health receives are those given in honor or memory of a loved one. We call these donations "tributes" and we're incredibly thankful for them, as well as the stories that accompany the gift.
A personalized donation page is a meaningful way to pay tribute to a loved one and can help create a place for your community to give back to Children's Health in memory of a loved one who has died.
We offer two options for families who want to create a memorial page:
We can create the page for you: If you prefer, you can complete this short online form, and we’ll take care of setting up the donation page. Once it’s ready, we will send you the login details and instructions for updating your password. From there, you’ll be able to personalize the page and view donations made in your loved one’s honor.
You can create the page yourself: To get started, visit our tribute fundraising page. You will click “Start Your Fundraiser” and then follow the steps. When you get to the step “create your page,” be sure to select, “I’m fundraising to honor someone else.” This makes setting a fundraising goal optional and allows you to dedicate the page to their memory.

What do I include in the obituary to direct donations to my Children’s Health memorial page?
In lieu of flowers, the family requests donations be made to Children’s Health in memory of (honoree’s Name). Contributions can be made at (insert memorial page link here).
Can I request that donations be directed to a specific fund or area in the hospital?
Unrestricted donations give Children’s Health the flexibility to deploy funds to areas of greatest need but, yes, you may request that donations raised be directed to a specific fund or area in the hospital. Please email
annualgiving@childrens.com if you are interested in designating funds.
How do I access my donation page?
Please click the “Sign In” button on the upper right corner of the page. Use the same credentials you used when you created the page. From there you can click “Manage” in the upper right corner of the page to access and edit you page.
Can I share my page on social media?
Yes, it’s a great way to inform family and friends of your loved one’s tribute page and guide them in making a donation in their memory.

Before pursuing this giving method, donors should be aware of the following:
Gifts are unrestricted by default: Unless a donor or family requests otherwise, memorial and tribute donations are directed to the Greatest Need at Children’s Health, giving the organization flexibility to apply funds where they are most needed. Contact annualgiving@childrens.com to request a specific designation.
Tax receipts go to the donor, not the honoree's family: Each donor who makes a tribute gift will receive their own tax acknowledgment. The family organizing the tribute page does not receive tax documentation for donations made by others.
Tribute pages are managed by the family: Once created, the tribute donation page is the family's responsibility to personalize, monitor, and maintain. Children's Health will provide login credentials and setup support.
Notification to family: Children's Health can notify the family when donations are received in honor or memory of a loved one. Families should clarify their notification preferences when setting up the page.
Fundraising goals are optional: When creating a tribute page, setting a fundraising goal is optional. Families should select 'I'm fundraising to honor someone else' to reflect the tribute nature of the campaign.

Key Rules and Guidelines
The following IRS rules, limits, and requirements apply to this giving method:
Tax deductibility: Tribute and memorial donations to Children's Medical Center Foundation are tax-deductible to the fullest extent permitted by law. Our Federal Tax ID is 75-2062015. Donors will receive a tax acknowledgment from the Foundation directly.
No goods or services: Because donors receive no goods or services in exchange for a tribute or memorial gift, the full contribution is generally deductible. Donors should consult their tax advisor for personal guidance.
Designated gifts: If a family requests that donations be directed to a specific fund, Children's Health must confirm it can accommodate the designation before promoting it. Contact annualgiving@childrens.com for approval.
IRS substantiation: For donations of $250 or more, donors are required by the IRS to retain a written acknowledgment from the organization as a condition of claiming a deduction.
Platform terms: Tribute fundraising pages are hosted on a third-party platform. Use of the platform is subject to that platform's terms of service, and Children's Health nor the Children’s Medical Center Foundation are not responsible for third-party platform policies.
